2.  Fill in the blank labeled b)  the number 3   and in blank labeled d). the number 20

3.  On page 2 where it says "DESIGNATED TRADING AGENT OR Introducing Broker.

     Enter the name Richard Henry into the blank.

4.  Send to FXDD either by fax or scan to the attention of Elena Kalaeva - New Accounts
